FREQUENTLY ASKED QUESTIONS:


Does someone on staff speak Spanish?

Yes! Our therapist is bilingual in Spanish and can offer every service in both English and Spanish.

Do you offer home or primary care physician office visits?

We will provide services outside of the practice on a case by case basis.

Can I bring my children to my counseling session?

Though we understand that sometimes it is difficult to arrange childcare we also understand that in order for therapy to be effective both the therapist and client must be present. We try to limit all distractions and therefore request that children not be brought to appointments unless they are a part of treatment.

What if I show up late to my appointment?

Because we know that life happens we will agree to see you if you arrive within 15 minutes. If you arrive after 15 minutes we will ask for you to reschedule your appointment. Please understand that if you arrive late you will not get additional time during that session; your session will end when it was scheduled to end.

What if I no-show to my appointment?

We value your time and hope that you will do the same for the staff at Revive. We understand that emergencies happen and therefore we will not terminate services unless you “no-show” more than 2 times. Please understand that full payment of service is still required for all "no-shows." If we decide to terminate services please know that we will do our best to provide you with other resources.

Do you accept insurance?

We do not accept insurance. All of our services are out of network. If you are participating in counseling we will gladly provide you with a superbill for you to submit to your insurance for reimbursement . If you request a superbill please be aware that we will have to provide a diagnosis code which will remain in your medical history.

Do you accept a sliding fee scale?

We provide limited slots at a discounted rate. If you are in a financial crisis we will consider a sliding fee scale with proof of income. We do our best to avoid turning anyone away because of finances. If we cannot meet your needs we will do our best to provide you with other resources.

Can I purchase a package of services?

Absolutely! We can create a package that best suits your needs.

Will you share my information with anyone?

Laws are in place to protect your privacy. The Health Insurance Portability and Accountability Act (HIPAA) contains a privacy rule that creates national standards to protect individuals' medical records and personal health information, including information about mental health.  All documentation will either be stored in a password protected online software or in a lock box. Please know that by law we are required to share certain information without consent. Please look at the next question for that information.

When can my information be shared without consent?

  • A therapist may disclose private information without consent in order to protect the patient or the public from serious harm. An example would be if a client discusses plans to attempt suicide or harm another person.

  • A therapist is required to report ongoing domestic violence, abuse or neglect of children, the elderly or people with disabilities.

  • A therapist may release information if they receive a court order. This could happen if a person's mental health is into question during legal proceedings. Please keep in mind that if our services are requested in court there will be a $300.00/ per hour fee.
